You are requesting an appointment with Melissa Korel (owner and lead artist) here at Pensacola PMU. No appointment with Melissa will be booked without first having received this form.

We ask that you allow a 24 (business) hour turn around time to receive and review your request. If you have not yet heard back from us after 24 (business) hours, please give us a call at 850-741-4071 or send a text to 850-686-0394 to inquire about setting up an appointment.

 Pensacola PMU does not offer online booking. It’s recommended to read over all FAQ’s, Policies, and Services pages before moving forward with filling out an ‘Appointment Request’ form.

Color Boost 'Touch Up' Appointment Request Form- Eyebrows, Eyeliner, Lip Blushing

If Melissa has done your previous PMU, please click on the ‘Color Boost Touch Up’ Appointment Request.’

If you have previous work done by anyone other than Melissa, you are still considered a ‘New Client’ and will need to fill out the ‘New Client Appointment Request’.
